The SIAM registration desk is located in the convention foyer of the Ballroom located on level B of the Cliff Lodge of the Snowbird Ski and Summer Resort. It is open during the following times:

Box lunches will be available for purchase @ $13.00 each (this covers cost of box lunch, tax and service charge,) Monday through Thursday. Box lunches will have sandwich, chips, cookie and drink. Snowbird does have two fast food/deli style restaurants that you can use if you would prefer. There will not be any refunds for box lunches on-site nor can we take orders for box lunches at the meeting. The box lunches must be ordered prior to the meeting with your registration.

There will be no prorated fees. No refunds will be issued after Wednesday, April 18, 2001. If your preregistration payment arrives at SIAM after the conference has started, that payment will be returned to you and your on-site registration will be processed.

The Conference Organizing Committee expects every speaker of a scheduled presentation to register and attend the conference. (SIAM would ask that the speaker(s) preregister so that all program materials are ready for the speaker(s) when they arrive and check in at the registration desk)

If it becomes necessary for a speaker to cancel his/her presentation, the speaker is expected to find an alternate presenter immediately, preferably one of the speaker�s co-authors. The speaker must inform the SIAM Conference Department immediately of any change to his/her scheduled presentation.

A �no-show� or cancelled presentation can cause serious inconvenience to the attendees and conference organizers. The committee thanks all speakers in advance for compliance with this request.

To qualify for the preregistration fee, return the Preregistration Form with payment to reach the SIAM office by Wednesday, April 18, 2001.

Any registration received at the SIAM office after Wednesday, April 18, 2001 will be subject to the full registration fee. We urge you to preregister and save the $60.00 difference.

There will be no prorated fees. Refunds will not be issued after Wednesday, April 18, 2001.
