Registration Fees

To register, complete the Registration Form and return it with your payment to SIAM. You can also register in the following ways:

We urge attendees to preregister and save! To qualify for the preregistration rate, the registration form and payment must be received at the SIAM office by Monday, May 5, 1997.

Registrations received at the SIAM office after Monday, May 5 will be subject to the difference between the preregistration and on-site registration fees. The difference will be charged to your credit card or collected from you on site.

Box Lunches

For lunch break, we are offering attendees the option of ordering a box lunch at $7.50 per person, per day. These box lunches must be preordered and prepaid. Please indicate on the Preregistration Form which days you are interested in ordering a box lunch, and include the total amount with your preregistration payment.

There will be no prorated fees. No refunds will be issued after Thursday, May 15, 1997. If your preregistration payment arrives at SIAM after the conference has started, payment will be returned to you and your on-site registration will be processed.

• Poster Session Reception I and II (Tuesday and Wednesday)

• Final Program and Abstract Booklet

SIAM accepts AMEX, MC, Visa, or check as a form of payment. When using credit cards, please indicate card type, account number and the expiration date on the registration form. Remember to include your signature on the appropriate line.

Non-SIAM Members

Non-SIAM members are encouraged to join SIAM to obtain the member rate for conference registration and enjoy the additional benefits of SIAM membership.
